Based on biometrics, claims data and clinical criteria (as defined by our physician-led Quality Committee), Kim identifies plan members eligible for her services and reaches out to primary care providers to coordinate referrals. For providers in AMG, she utilizes the Patient Case functionality in Athena.
It is especially helpful if providers are able to make a “warm handoff” to Kim via a 1:1 discussion during an appointment, phone call from the practice to the patient, or sending an introduction letter to increase that patient’s openness to services. After an initial assessment and connection to services, Kim continues to follow her patients, monitoring for adherence and reinforcing success.
