Are financial concerns preventing your patient from having a
screening for diabetic retinal disease?
Last year, our network’s screening rate for diabetic retinopathy was 63%, but we can do better! If a diabetic patient cannot afford an eye exam, please contact our case management team. Case managers will work with the Department of Social Services, the Augusta Regional Clinic, and the Lions Club to provide coverage for the cost of the exam, lenses, and frames.
Referrals can be made in a variety of ways:
- AMG Providers: Use athena to make a referral to your assigned case manager (search for “Augusta Care Partners (Care Manager Name)”). If your practice does not have a defined case manager, you can make a general referral to “Augusta Care Partners”. Please remember to include a note about what services need to be provided.
- Non-AMG Providers: Call / email your case manager directly. If your practice does not have a defined case manager, call (540) 245-7570 or email our case management assistant, Wendy Hamilton, at whamilton@augustahealth.com.
