Here to Help: ACP’s Case Management Team

Augusta Care Partners has a team of 5 RN case managers and a case management assistant dedicated to coordinating care for our 11,000+ Medicare and Augusta Health health plan patients. Case managers are assigned to patients based on where the patients access primary care. All network PCPs and some high-volume specialty providers have a defined case manager for their practices (see chart below).

To Make A Referral

Any practice within the ACP network can make a case management referral. Common reasons for referral include:

• Difficulty Affording Medications
• Suspected Abuse/Neglect
• Disease Management
• Financial Concerns
• Transportation Issues
• Frequent Hospitalizations
• High ER / Urgent Care Utilization

Case management services are free of charge to patients, and referrals can be made in a variety of ways:

• AMG Providers: Use athena to make a referral to your assigned case manager (search for “Augusta Care Partners (Care Manager Name)”). If your practice does not have a defined case manager, you can make a general referral to “Augusta Care Partners”. Please remember to include a note about what services need to be provided (ex. food security issues, medication access, transportation, etc.)
• Non-AMG Providers: Call / email your case manager directly. If your practice does not have a defined case manager, call (540) 245-7570 or email our case management assistant, Wendy Hamilton, at whamilton@augustahealth.com.

Community Resource List

The Augusta Care Partners case management team put together an updated list of community resources, and we wanted to share it with you! This information can be useful if a case manager is not readily available or the patient simply wants information about local services. While not totally inclusive of every resource in the area, it covers many of the high-need services that we commonly access for our patients.
